Please follow the steps below…

1. Our forms are now PDF fillable! Please open in Adobe Reader and use the “fill & sign” feature to complete. They can also still be printed out and completed by hand.
2. Please find the camp(s) for each of your children.
3. Download all forms needed for each camp.  Note:  If your child is attending multiple camps (weeks), we only need the Medical and Northbrook forms to be completed once.
4. Please complete all forms and save as a PDF.  Please name the files using your child’s first name, last name and form name in the file name.  For example, AlexaHartMedical.pdf, AlexaHartNorthbrook.pdf, or AlexaHartGAFLiability.pdf.
5. Please email all of your child(rens) forms in one email back to us.
6. All forms that are not in .pdf format or are incomplete will be returned for resubmission.
7. All forms are due to BRC no later than May 24, 2024.
